Southern launches electronic COPD checklist

A new electronic standardised discharge process has been launched for patients admitted to Dunedin Hospital with acute chronic obstructive pulmonary disease.
Southern DHB says the ‘Click to Tick’ electronic COPD checklist means patients have more integrated care between hospital and general practice, rehabilitation and community-based services.
The checklist is embedded in the online discharge summary and accessed via the clinical portal, Health Connect South. While launched in Ward 7A at Dunedin Hospital, it is available for use across all the district health board’s hospitals for patients in internal medicine and respiratory wards.
Where COPD is the primary diagnosis, a box pops up and asks the clinician doing the discharge to complete a series of seven to eight tasks.
These include ensuring the patient gets referrals and advice for correct medicine and inhaler use, pulmonary rehabilitation options and information about smoking cessation, healthy homes and advance care planning.
Booking a fully funded follow-up general practice appointment one to two weeks post discharge is also part of the ‘Click to Tick’ process.
The list is available electronically to GPs immediately on discharge and highlights relevant points to the GP for further attention at the follow-up appointment.
Southern DHB respiratory specialist Jack Dummer was part of team that developed the COPD discharge checklist.
He says that while it is not mandatory, having the list available electronically makes it easy for clinicians to “get the simple things done” while the patient is still in their care.
“Our intention is to promote its use across the district over the next six to 12 months,” he tells eHealthNews.nz.
The team will also monitor its use and whether it is making a difference for patients.
