The Foundation is the official charity of Waitematā DHB. The fund will support local level response to COVID-19 across West Auckland, North Shore and Rodney. The focus is to establish remote patient monitoring for those in the community with underlying health conditions.
The initiative, led by Waitematā DHB's Institute for Innovation and Improvement, will allow staff at North Shore and Waitakere hospitals to monitor the most vulnerable in the community in their own homes, keeping them out of the hospital environment.
"COVID-19 most severely impacts the elderly and those with underlying health conditions. This Action Fund supports the expert efforts of our local health services as they develop targeted responses for the most vulnerable in our community," Tim Edmonds, Well Foundation CEO says.
How will donations be used?
Focus:
- Waitematā DHB is the largest District Health Board in New Zealand with a population of 630,000 people across the North Shore, West Auckland, and Rodney.
- Approximately 70,000 in our community are over the age of 70.
- Donations will be used to support the targeted response for those most vulnerable in our community, including older people and those with underlying health conditions.
- A significant challenge faced by North Shore Hospital and Waitākere Hospital is that vulnerable patients still need access to acute and chronic care, unrelated to COVID-19.
- We are now facing the reality that new models of care, outside of our hospitals, are needed. The development of virtual clinics and community-centred care for our most vulnerable is an urgent priority.
