SynopsisMaking connections in an integrated health neighbourhood: who, what, how and what for?Digital health connections require an integrated health neighbourhood (IHN) to provide a context and to give meaning to the work. The IHN is a network of primary and secondary health services established around a local hospital. The underlying rationale is “people identify with their local GP and their local hospital” and the overarching conceptual framework is the Chronic Care Model. What do we need to connect and integrate? This includes data and knowledge, information systems and tools, services and delivery systems, and people and professions. How? That’s the current holy grail of health informatics – interoperability – and the variety of tools and standards available to address the implementation challenge. What for? The vision is an activated patient and community interacting meaningfully and effectively with an activated clinician and healthcare team to achieve and sustain good functional clinical and population health outcomes. Sustainability requires citizen engagement, public-private partnerships, good corporate social responsibility and social business approaches to software and community development and technology diffusion.
