Synopsis

Walking Backwards into the Future

We often hear the adage ‘It takes a community to raise a child’ but we have been slow to acknowledge that the best environment to nurture our whānau ‘to get well, live well and stay well’, is through active involvement from differenct agencies and professions.

Whānau ora through a coaltion of providers working in a Collective Impact Alliance model of care is creating more meaningful outcomes for whānau whilst having an eye on the social and economic return on investment.

This return on investment may be improved by simply making better use of what we have as we refocus on building health literacy and self- determination in our communities .........

Biography

Jo-Ann Jacobson
Health Lead
Whānau Tahi Limited

Jo-Ann Jacobson has been in the role of Market Lead for Whānau Tahi Limited for the past 10 months.

Whānau Tahi’s moemoea is for vital wellness data and care plans to be shared across health, education, whānau ora, and social service providers to ensure each support person understands the unique needs and history of the whānau on each step of their journey, creating more meaningful outcomes for whānau whilst having an eye on quantifying the social and economic return on investment.

Jacobson says “I feel I have found my spiritual home in Whānau Tahi”. Since her early days in the sector she has driven her passion for integrated and innovative care in a number of executive roles across the spectrum of public, private, commercial and Not for Profits including National Health Lead, Spark NZ and CIO, Hawkes Bay District Health Board.

She received a post graduate degree in Human Resource Management from the University of Auckland, and a bachelor's degree from Waikato University.
