Person-Centred Health System Award Application

By submitting this application you confirm that you are eligible for the award and agree to the following terms and conditions.

1. The article must be unpublished, original, entirely the author’s own work, and must not infringe copyright or any other rights of any third party.
2. The author must be working in the New Zealand health sector. (Authors who are studying health informatics at a NZ college or university can apply for a Rising Stars Award.)
3. Members of the HINZ Executive and employees of the Ministry of Health are not eligible to apply.
4. The author agrees for the article to be published in Health Care and Informatics Review Online with the following disclaimer:

5. The author grants non-exclusive rights to the Ministry of Health to reproduce the article in full or part form with acknowledgement of his/her authorship.
6. The author agrees to obtain the Ministry of Health’s prior written consent, not to be unreasonably withheld, before taking on any of his/her own press or publicity activity relating to their involvement in the award.
7. If the author reproduces the article in full or part form, he/she agrees to include the following statement:

8. The author agrees to the article being copy-edited for journal editorial style and notes that he/she will receive final approval of the article prior to publication.
9. HINZ or the Ministry of Health reserves the right to cancel the awards programmes at any stage, if deemed necessary in its opinion, and if circumstances arise outside of its control.
10. Winners will be contacted personally by email or by telephone.
11. The judging will be fair and independent of commercial interests. The judging panel will be appointed by the HINZ Executive and will include a representative from the Ministry of Health.
12. The judges’ decision is final and no correspondence will be entered into.
